Employers generally have no warning when an employee may approach a manager or HR representative to file a complaint. In fact, most often the employee will simply be coming in to have a discussion, which turns into a complaint of wrongdoing that requires an investigation. For this reason, the information used to complete the incident report may be provided by the complainant at the time the incident report is completed, or may be reconstructed from notes taken at the initial meeting by the HR representative or manager who met with the employee. The important factor is that all of the information is complete and accurately reflects the initial complaint. Further details on the incident will be obtained through the interview process.
